Form Instructions

• Requests must be submitted by a chapter official.
• Chapter contact will receive an email when this form is submitted. The chapter contact must validate this request as described in the email before the NISOA Executive Board will consider the request.
• NISOA may request receipts or other forms of expense validation (estimates) prior to request approval.
• Chapters may have only one active grant request at a time.
• All form fields are required.
